Physicians have told me that these ways of organizing medical thinking truly revolutionized the practice of medicine &#8211; I&#8217;d welcome discussion in the comments.<\/p>\n<p><!--more-->Well,\u00a0someone unearthed <a href=\"https:\/\/www.youtube.com\/watch?v=qMsPXSMTpFI\" target=\"_blank\">this priceless video<\/a> of him lecturing about it at Emory University when it was brand new &#8211; \u00a0in 1971,\u00a0forty-three years ago. (Check out the snazzy overhead projector, the microphone strapped to his chest!) If you don&#8217;t know how medicine was practiced\u00a0<em>before\u00a0<\/em>this unique idea, you may be aghast at the first few minutes. Believe it or not, the medical record was not at all organized by the patient&#8217;s problems; listen to the mess\u00a0that resulted.<\/p>\n<p>Then he gets into how impossible it will be to improve medicine if we don&#8217;t get systematic about how we define problems and systematic about our approaches to them. Many of the points he made are still applicable today.\u00a0His current book is\u00a0<em>Medicine in Denial<\/em>, a title that\u00a0will come of no surprise to you if you make it through all 54 minutes of this talk.<\/p>\n<p>Dr. Weed is 80 now, and lives in Vermont, where I gave a speech\u00a0yesterday. Gilles cited\u00a0Weed a couple of times, and also makes a brilliant point about e-patients <em>collaborating with<\/em> experts, not replacing them.<\/li>\n<li>The Amazon page for\u00a0<em><a href=\"http:\/\/www.amazon.com\/Medicine-Denial-Lawrence-L-Weed\/dp\/1456417061\" target=\"_blank\">Medicine in Denial<\/a>.<\/em> The description says he\u00a0&#8220;more than 50 years ago, originated influential standards for organizing medical records. He applied these standards (known as the problem-oriented medical record) in designing electronic record systems.
