Setting the stage for a data disaster, part 2"},"content":{"rendered":"<p>What if you hired a financial manager to handle your money &#8211; a trusted professional, like that guy above &#8211; then discovered they didn&#8217;t know what they were doing, and your records were a mess? How would you feel?<\/p>\n<p>That&#8217;s pretty much what happened to me ten years ago this month, except it wasn&#8217;t my money, it was my health data.<\/p>\n<p>The errors were so bad, and the lessons were so surprising, eye-opening, and important that when I blogged it, to my surprise the <em>Boston Globe<\/em>\u00a0called and put the story on page one.<\/p>\n<p>That was ten years ago this week, and to understand what&#8217;s happening in health data right now, it helps to know how we got here. This post is the next in a series reviewing that history and pointing the way forward.<\/p>\n<h3>What happened &#8211; elevator version:<\/h3>\n<p>I had tried to use the long-dead <a href=\"https:\/\/www.hitechanswers.net\/the-end-of-google-health\/\" target=\"_blank\" rel=\"noopener\">Google Health<\/a> to look at my health data and discovered it was full of garbage: false medication warnings, false diagnoses, false billing codes, all kinds of things.<\/p>\n<p>Google Health was a primitive attempt to collect information from multiple hospitals &#8211; primitive enough that <a href=\"https:\/\/www.hitechanswers.net\/the-end-of-google-health\/\" target=\"_blank\" rel=\"noopener\">it died in 2011<\/a>, but it was potent enough to crack open the scandalous issues of errors in our records, bad software for managing our data &#8230; and complete inability of medical record systems to let you move your data to anywhere you need it.<\/p>\n<p>(Don&#8217;t blame Google for this &#8211; the need is real but the work is hard. Just last week <a href=\"https:\/\/www.mobihealthnews.com\/content\/microsoft-will-officially-shut-down-healthvault-later-year\" target=\"_blank\" rel=\"noopener\">Microsoft announced<\/a> that it too is killing its same-era product HealthVault.)<\/p>\n<h3>FHIR is on the horizon. This is good.<\/h3>\n<p>The portability issue is addressed by a new health data interface called <a href=\"https:\/\/thehealthcareblog.com\/blog\/2016\/01\/20\/fhir-the-last-best-chance-to-achieve-interoperability\/\" target=\"_blank\" rel=\"noopener\">FHIR<\/a>\u00a0(pronounced fire), which is in healthcare news constantly these days. (<a href=\"https:\/\/www.google.com\/search?q=fhir+news&amp;tbs=sbd:1,qdr:m&amp;tbm=nws&amp;source=lnt&amp;sa=X&amp;ved=0ahUKEwiPlJXTg8nhAhWGT98KHYHPCYUQpwUIHw&amp;biw=1431&amp;bih=664&amp;dpr=1\" target=\"_blank\" rel=\"noopener\">Here&#8217;s a Google News search<\/a> for &#8220;FHIR news&#8221; in the past month &#8211; over 100 articles.) Why? Because hospitals and other companies involved in health data\u00a0<strong>are actually starting to use FHIR<\/strong>, and it looks like it may be the long-awaited &#8220;<a href=\"http:\/\/bit.ly\/healthspigot\" target=\"_blank\" rel=\"noopener\">health data spigot<\/a>&#8221; that we empowered, engaged e-patients have sought for years.<\/p>\n<h3>Do portable medical records matter? Only if you&#8217;re sick, and especially if you&#8217;re in trouble.<\/h3>\n<p>A fundamental tenet of participatory medicine is that <strong>you need to know what&#8217;s in your medical record<\/strong> (or your child&#8217;s, or your parent&#8217;s, or..) because <strong>the data is likely to have errors\u00a0<\/strong>(<a href=\"https:\/\/participatorymedicine.org\/epatients\/2014\/06\/wall-street-journal-on-patient-access-health-care-providers-want-patients-to-read-medical-records-spot-errors.html\" target=\"_blank\" rel=\"noopener\">Wall Street Journal 2014<\/a>), and <strong>that&#8217;ll mess up any clinician.\u00a0<\/strong>Docs have thousands of people to keep straight, and <a href=\"https:\/\/khn.org\/news\/death-by-a-thousand-clicks\/\" target=\"_blank\" rel=\"noopener\">their computers are generally crappy<\/a>. Be responsible &#8211; help!<\/p>\n<p>Another major reason is that today when you show up at a new care facility &#8211; like, when your family member has a real problem and gets referred elsewhere &#8211; the chances are very good that the new place won&#8217;t have all the relevant info, which will mess them up, too.\u00a0<strong>Only you\u00a0<\/strong>are in a position to know &#8220;all the truths&#8221; from past providers. So you really should have all the information.<\/p>\n<h3>But collecting the data has been really hard. That&#8217;s where FHIR comes in.<\/h3>\n<p>A major reason access has been hard is that <strong>there&#8217;s never been a standard interface<\/strong>\u00a0that systems can use to exchange data, or for you to get it out. Without a standard interface, our (your) data has largely been in isolated computers, the same as the world was before the internet was born: plenty of valuable knowledge, but nobody can get at it.<\/p>\n<p>So\u00a0<a href=\"https:\/\/participatorymedicine.org\/epatients\/2019\/04\/its-the-10th-anniversary-of-trying-to-get-our-health-data-im-putting-my-data-in-google-and-healthvault.html\" target=\"_blank\" rel=\"noopener\">yesterday I blogged<\/a> the first of a series of updates to our blog posts on the subject in 2009. That was on the importance of making the most of our data.
