She prints a fresh copy to take to every doctor visit, so they&#8217;ll always have it up to date and won&#8217;t have to go digging.<\/p>\n<p>We&#8217;ve never discussed topics like &#8220;the importance of med reconciliation,&#8221; but when she sent it I got a little insight into who I am.\u00a0:\u2013)<\/p>\n<ul> <a href=\"https:\/\/participatorymedicine.org\/epatients\/wp-content\/uploads\/sites\/3\/2010\/04\/davemom.png\" data-rel=\"lightbox-image-0\" data-rl_title=\"\" data-rl_caption=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-5358\" style=\"margin-left: 10px; border: 1px solid black;\" title=\"'s mom takes to every doctor visit\" src=\"https:\/\/participatorymedicine.org\/epatients\/wp-content\/uploads\/sites\/3\/2010\/04\/davemom-214x300.png\" alt=\"What Dave's mom takes to every doctor visit\" width=\"214\" height=\"300\" srcset=\"https:\/\/participatorymedicine.org\/epatients\/wp-content\/uploads\/sites\/3\/2010\/04\/davemom-214x300.png 214w, https:\/\/participatorymedicine.org\/epatients\/wp-content\/uploads\/sites\/3\/2010\/04\/davemom.png 555w\" sizes=\"auto, (max-width: 214px) 100vw, 214px\" \/><\/a><\/p>\n<li>Medications (prescription and over-the-counter), including dosage and when she takes them<\/li>\n<li>Who prescribed them and for what condition<\/li>\n<li>Full contact info for every care provider including her acupuncturist<\/li>\n<li>Contact info for local family members and neighbors, including who has a key to the house and the garage door code<\/li>\n<\/ul>\n<p>She keeps a copy on the fridge, too, a trick she learned from EMTs during Dad&#8217;s declining years. If they get called for an emergency and you&#8217;re unconscious when they arrive, it can be important. She adds that she &#8220;Should, but don&#8217;t, also have there the ORIGINAL of any orders like\u00a0a DNR (Do Not  Resuscitate), to make their job easier in the event of&#8230;..&#8221;<\/p>\n<p>Gosh, Mom, you make me proud! \u2665<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>I was talking to Mom (age 80) the other night, and she mentioned a document she updates regularly.
